Specific, measurable, connected

A care input is not a task. It is a designed response to a named need. Each one should describe what you will actually do, how often, and how you will know whether it is making a difference.

Every month, return to this plan and ask: are these inputs still meeting the original needs? Have the needs changed? Are any inputs ready to be retired or adjusted?
